Use the list of Licensing Forms to select the application form you require.

 

The forms are currently available in Microsoft Word format, and once completed can be sent either via e-mail or regular post to the Licensing Section.  Please see the Licensing contact details.

 

If the form you want is not listed then it may be that responsibility for that licence falls within another service area.  Please telephone the Licensing Section on 01743 281376 during office hours who will be able to advise you.  

 

Outside office hours please leave a telephone message or send an e-mail to licensing@shrewsbury.gov.uk detailing the nature of your enquiry and a telephone number, home or e-mail address and a member of staff will contact you with the information you require.
